She is actually the protagonists childhood friend, he dies when he is little due to drowning, she saves him from a shinigami came to claim his soul, under the condition she herself would become one and give her soul to him. the route is about kohaku coming to this realization and the protagonist, at the end they find out and she comes back to life at the exchange of Makotos life, but the shinigami that made kohaku one... Izayoi feels that it was her

mistake in the first place by the involvement in human lives and gives Makotos soul back, tada they both live happy ever after and all that jazz
