- At least one of the endings involves the death of said character

 

I also liked Yume Miru Kusuri! These are the only two e-games I've played so far.

Also "Im Gonna Serve you 4" has MC terminal illness and some emotional endings, but it's a nukige.

Katawa Shoujo might be in your ballpark except..

 

[spoiler=Slight spoilers]The death of the protag isn't really an ending, it's shown when you don't get on any of the routes and you end up on the kenji "route"

vndb.org is a database of visual novels/eroge that is very helpful. This link will give you a list of games with terminal illness in them. That list hides spoilers, but you should be able to easily find the option just above the list to show spoilers. It has many other themes you can look for too.

Ryu's choices are more to funny part while desi's choices are more heavy....

you can go for Utawa also...

but for more of death involving sick chara...

its more rare...but either Moon and Giniro should do the trick if i rmb their plots correctly

Wind and SnowDrops are ok too
