These are the only two e-games I've played so far. Private Nurse has everything you want. Also "Im Gonna Serve you 4" has MC terminal illness and some emotional endings, but it's a nukige. [spoiler=Slight spoilers]The death of the protag isn't really an ending, it's shown when you don't get on any of the routes and you end up on the kenji "route" Also, this one is probably a spoiler. This link will give you a list of games with terminal illness in them. That list hides spoilers, but you should be able to easily find the option just above the list to show spoilers.
